What is PACE?
PACE (Program of All-Inclusive Care for the Elderly) is a personalized health plan and community-based provider of coordinated care, support services and social engagement.
Who Can Enroll in PACE?
Participants must live in our service area, be at least 55 years old, and eligible for nursing home care but able to live safely at home or in the community with support from Florida PACE Centers.
What is the Cost of PACE?
If you have Medicaid only or are eligible for both Medicare and Medicaid, you may receive care at no cost. If you have Medicare only, you will pay a monthly premium for Florida PACE Center services. A private pay option is also available.
What Should Caregivers Know?
Florida PACE Centers can help reduce the stress and pressure on family caregivers. We provide quality healthcare, coordinate appointments, manage medications, arrange transportation, and offer meals & social engagement.
