How much does PACE cost?
Medicaid
If you are on Medicaid and qualify for PACE, Medicaid will cover all of your costs.
Medicaid + Medicare
If you have Medicaid and Medicare and qualify for PACE, the two programs will combine to cover all of your costs.
Medicare Only
If you are on Medicare only and qualify for PACE, you will pay a small monthly premium for PACE services. It’s an affordable and practical way to meet all of your healthcare needs.

1. Assessment
Complete an assessment for eligibility for services. Call us at 786.933.7223 or TTY # 800.955.8771 to get started.

2. Documentation
Gather your financial documents including a recent bank statement, copies of any retirement funds, social security, disability, pension and other monthly income. Bring these when you meet with your PACE enrollment specialist.

3. Meet
We will arrange virtual or in-person meetings with the healthcare team at your Florida PACE Center as part of your assessment.

4. Care Plan
If you are eligible for PACE, we will get the final paperwork completed and begin working with you on your plan of care.
